By enrolling my child/children in any activities, including but not limited to summer camps, spring break camps, fall break camps, after-school events, parties, tabletop role-playing games (Dungeons & Dragons), video game events, STEAM programs, or outdoor activities hosted by Moonshot Games Noblesville or Moonshot Games Plainfield (hereinafter referred to as "Moonshot"), I acknowledge and agree to the following terms:

1. Assumption of Risk & Waiver of Liability

I understand that participation in Moonshot activities, whether held on-site at Moonshot Games locations or at third-party venues, involves inherent risks, including but not limited to:

  • Physical activity and movement-related risks
  • Use of game-related materials, terrain setups, miniatures, dice, or electronic equipment
  • Outdoor environmental factors during activities held in nearby parks or other locations
  • Interactions with other participants and the general public

I assume all risks associated with my child’s participation and release, waive, and hold harmless Moonshot Games, its owners, staff, volunteers, and affiliates from any claims, damages, or liabilities that may arise from my child’s involvement in these activities, including but not limited to injury, illness, loss of personal property, or emotional distress.

I acknowledge that role-playing games like Dungeons & Dragons involve fictional scenarios, including in-game character battles, conflicts, and character "death." I understand these are part of the storytelling experience and that no real harm comes to any participant. I will discuss this with my child before participation to ensure they are comfortable with these elements of the game.

2. Medical Authorization & Emergency Care

I grant permission for Moonshot staff to take whatever steps necessary to obtain emergency medical care for my child if warranted, including but not limited to:

  • Administering minor first aid (e.g., bandages, ice packs, antiseptic wipes).
  • Attempting to contact a parent/guardian or emergency contact.
  • If a parent/guardian cannot be reached in a timely manner, Moonshot staff may:
    • Call a physician.
    • Call an ambulance.
    • Transport the child to the nearest medical facility, if necessary, with a staff member present.

I understand that any expenses incurred for emergency medical services are the sole responsibility of my child’s family.

3. Code of Conduct & Dismissal Policy

I acknowledge that my child is expected to follow Moonshot’s rules and conduct guidelines, including:

  • Respecting staff, fellow participants, and property
  • Following all safety rules and instructions
  • Maintaining a positive and inclusive attitude

If my child engages in disruptive, unsafe, or inappropriate behavior, Moonshot reserves the right to dismiss them from the activity without a refund.

4. Media & Promotional Release

I grant permission for my child to be photographed, recorded, or included in promotional materials related to Moonshot Games events. This may include but is not limited to:

  • Moonshotgamestore.com
  • YouTube, TikTok, Instagram, Facebook, and other social media platforms
  • Promotional flyers, advertisements, or event documentation

If I do not want my child included in promotional materials, I will notify Moonshot Games in writing before participation.

5. Parental Acknowledgment & Consent

I affirm that my child has no medical, emotional, or social conditions that would make participation unsafe or unwise. If any special accommodations are needed, I will communicate them with Moonshot staff in advance.
